New Patient - Need Provider

We have seen a significant increase in the number of people who, like you, are looking to make NOHN their medical home. This increase in volume — coupled with limitations in staffing resources and provider schedules — has resulted in a temporary 8-12 week delay in processing new patient requests. In light of this expected delay, please do not use this online form for urgent requests. If you have a life-threatening medical condition, call 911 or visit the ER. 

Please Note: At this time, we are prioritizing obstetrics and pediatrics patients, as well as community members who currently do not have a primary care provider. We are working hard to increase access by improving processes and recruiting new providers to the area.

Please complete the form below and we will forward your request to our New Patient Liaison. If you have several family members wanting to establish care, please fill out a form for each person.

We appreciate your patience as we review, queue and process your request. Our New Patient Liaison will contact you by phone to complete your registration, schedule you for your New Patient appointment, and share information with you about all the services available to you as a NOHN patient.
