Chronic conditions are ongoing medical problems that must be managed effectively in a partnership between the health care team and the patient to maintain the best possible health. Examples include diabetes, high blood pressure, heart disease, depression, and others. Federal regulations enable Medicare to pay for chronic care management.
What is chronic care management?
If you have Medicare, or Medicare and Medicaid, and have two or more chronic conditions, then Medicare is offering chronic care management services to help you manage your health and spend more time doing the things you love.
Chronic conditions include:
- Alzheimer’s disease and related dementia
- Arthritis (osteoarthritis and rheumatoid)
- Asthma
- Atrial fibrillation
- Autism spectrum disorders
- Cancer
- Cardiovascular disease
- Chronic obstructive pulmonary disease
- Depression
- Diabetes
- Hypertension
- Infectious diseases such as HIV/AIDS
What are the benefits of chronic care management?
Regular chronic care management means you can better manage your care and spend more time focusing on your health. Chronic care management can help you work towards your health and quality of life goals. You’ll get personal attention and help, and you can feel secure knowing you’ll gain a comprehensive care plan.
Services may include:
- At least 20 minutes of chronic care management services per month
- Personalized assistance from a dedicated health care professional who will work with you to create your care plan
- Coordination of care between your pharmacy, specialists, testing centers, hospitals, and more
- Phone check-ins between visits to keep you on track
- 24/7 emergency access to a health care professional
- Expert assistance with setting and meeting your health goals
What do you need to know before signing up?
Understand that this care requires you to pay a minimal amount (your Medicare coinsurance amount) to your primary care practice each month that you receive chronic care management. The service is also subject to your Medicare deductible.
You must sign an agreement to receive this type of chronic care management.
