North Olympic Healthcare Network (NOHN) has received three American Heart Association outpatient program achievement awards for reducing the risk of heart disease and stroke among its adult patient population by improving high cholesterol, blood pressure management, and type 2 diabetes. The awards recognize a commitment to following the latest evidence-based care guidelines.
NOHN received the following achievement awards:
Change. Control. Cholesterol™ Gold recognition presented by the American Heart Association for its commitment to improving ASCVD (Atherosclerotic Cardiovascular Disease) risk assessment AND having 70% or more of its adult, at-risk ASCVD patient population appropriately managed with statin therapy.
Target: BP™ Gold+ recognition jointly presented by the American Heart Association and the American Medical Association for demonstrating a commitment to measurement accuracy AND controlling high blood pressure in at least 70% of its adult patient population.
Target: Type 2 Diabetes℠ Gold recognition presented by the American Heart Association for meeting specified thresholds for two clinical measures while improving quality of care through awareness, detection, and management of type 2 diabetes by educating and empowering patients with evidence-based information and tools.
The 2025 awards are based on clinical outcomes for the January 1 – December 31, 2024 reporting period.
