Help in Accessing Community Resources
Our Community Resource Navigators help patients address any non-medical barriers to their healthcare. Experts in our local community resources and how to access them, our navigators work with providers and patients to remove the things that get in the way of being one’s healthiest self by rallying our rich web of community resources for the task.
Through assistance with obtaining affordable health insurance, to help with transportation to medical appointments, food, housing, interpretive services, home care, and many other local services, our navigators can help patients meet their health goals by connecting them with the resources they need in order to focus on getting and staying healthy.
Health Insurance Enrollment: Need health insurance? Meet with a navigator to complete the Washington Healthplanfinder application, review eligibility results, and assist with submitting documents requested by the Washington Health Benefit Exchange.
Transportation: Missing appointments due to lack of transportation? NOHN provides transportation assistance such as bus tickets, taxi vouchers, and gas cards. The navigator can also connect you to alternative transportation options available in our community.
Housing: If you are unsure about where to begin looking for housing, our navigators can help. We can help connect you with local agencies that offer housing assistance in our community.
Emergency Assistance: Life happens, and sometimes a little extra help is needed to meet basic needs. We are fortunate to live in a community that has organizations ready to help. Our navigators can connect you to different organizations that can help meet your emergent needs.
Family Support Services: “It takes a village” is what they say. Our community is home to several agencies that offer varying levels of support for families. Services range from clothing closets to home visiting and parenting support.
Senior Support Services: Do you have questions about resources to meet your needs during your golden years? Ask our navigators for information about home care agencies, long-term care options, Medicare assistance, senior activities, transportation, and additional resources.
