Sports Physical Clinic – August 16
We’re offering a one-day Sports Physical Clinic for all K-12 and college students on Saturday, August 16* at our Eastside Health Center at 1026 E. 1st Street, Port Angeles. The clinic is open to anyone for a $100 flat fee, however, NOHN recommends scheduling a sports physical as part of an annual Well Child Check with your primary care provider (PCP) whenever possible, as this option is covered by most insurances once a year.**
How to schedule: Call (360) 452-7891 x3002, x2822, or x2831 and ask to schedule a sports physical. Appointments are limited, so early scheduling is encouraged.
Cost: Flat fee option: $100 for sports physical only. Insurance will not be billed. Insurance option (NOHN patients only): If a student is due for a Well Child Check, we will schedule it to include a sports physical and bill their insurance.
What to bring: Students must bring the required physical forms from their school.
