Need a school sports physical for your child?
NOHN patients can schedule an appointment during regular clinic hours for a $100 fee or as part of a well child check if appropriate. If you have CHPW insurance, both a sports physical and a well child exam are covered. Call (360) 452-7891 to schedule or for more information.
Sports physicals are also available to all PASD students on our Mobile Health Clinic during visits to each school. Simply fill out a Registration and Consent form and hand it in to your coach, nurse, counselor or teacher. You will be called to schedule the very next available appointment. Parents, please complete the Agreement to Pay for Healthcare Services and send with your child if you will not be present with your child during their Mobile Health Clinic visit.
