It is our pleasure at NOHN to support you as a patient. As you may know, our practice is a member of an Accountable Care Organization (ACO). An ACO helps coordinate high-quality care to Medicare patients and save costs. Because of this, it’s important for Medicare to know you are a NOHN patient.
Choosing Your Primary Clinician
Please consider taking a few minutes to update your primary care clinician selection on Medicare.gov. This is the clinician you feel handles most of your care. Please review the following resources to help you complete this process:

- Voluntary Alignment: How to Choose Your Primary Health Care Clinician on Medicare.gov – a step-by-step instruction guide on how to choose your clinician. The step-by-step instructions guide you through how to complete the process on Medicare.gov. It includes how to review your “favorite” clinician(s) and choose your primary clinician in Medicare’s system.
We would appreciate your help in completing this “Voluntary Alignment” process. This process is completely optional (not required). Whether you choose to complete the process or not, your Medicare benefits will not change in any way. You can still see any clinician of your choosing.
IMPORTANT: You do not need an email address to establish an account with Medicare.gov. However, access to the Internet and your Medicare card are required.
If you need extra help with the process, we are happy to assist you in the office. When you come in for your next appointment, our staff are available to help before or after your visit. Please allow 10-15 minutes at the beginning or end of your appointment to complete this process. Don’t forget to have your Medicare card with you.
Please do not call us solely for assistance with this process. Our staff likely will not be able to assist you over the phone. Instead, please review the above resources and step-by-step instructions. As mentioned, you can seek in-person assistance from our office staff before or after your next visit.
Remember that you will need to complete this process any time you switch primary clinicians to ensure your information is up to date in the Medicare system.
