NOHN Achieves NCQA Recognition

National Committee for Quality Assurance Patient-Centered Medical Home logoNorth Olympic Healthcare Network has achieved National Committee for Quality Assurance (NCQA) recognition for meeting the Patient-Centered Medical Home (PCMH) standards of care. The NCQA PCMH standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communications and patient involvement. Research shows that NCQA-recognized medical homes deliver higher quality care, help control costs and improve the patient experience.

What does that mean for you as a patient? It means that our provider-led care teams deliver comprehensive, coordinated, accessible, evidence-based care and self-management support.

Your Choice: Choosing the right provider for you is an important step in building a strong patient-provider relationship. Your provider and care team will help you coordinate all your care to keep you at your healthiest. When you become a patient, a representative from our Patient Services team will discuss your available options and assist you in finding a provider that best meets your needs.

Comprehensive Care: Our full-spectrum approach to medical care means we focus on the whole person, and the whole family. Whole-person healthcare – including medical, dental and behavioral health care, as well as prevention and wellness, acute care and chronic care – is available for patients of all ages. View more information about our comprehensive array of available services by visiting the Services section of our website.

Coordinated Care: Your care team includes a provider, nurses, medical assistants and many other healthcare professionals that get to know you and your healthcare needs. The team is responsible for providing for all of your health care needs, arranging care with specialists as necessary, and coordinating care across the health care system to ensure that you receive the right care at the right place at the right time.

Accessible Care: We’re available at (360) 452-7891 (call or text) during business hours. You may also message your care team via MyChart. For non-urgent clinical advice after hours, please call (360) 452-7891 to be connected to available clinical resources as appropriate.  North Olympic Healthcare Network is an equal opportunity provider and employer, and we serve all patients regardless of their ability to pay.

Evidence-Based Care: We utilize evidence-based protocols and ongoing quality measurement and improvement to provide you with the best possible care outcomes and experiences.
